Chain of Custody
A Chain of Custody (COC) needs to be submitted with every sample. Please make sure you include project number, contact information including email address for reporting, and sign and date the COC when you drop off/mail your samples.
Credit Application
We ask that all new customers fill out a credit application so that we can process you and add you to our system. This will allow us to bill you instead of paying when you drop off samples.
